IMMIGRATION MEDICAL EXAMS
Randolph Rosarion M.D. is designated by the U.S. Citizenship and Immigration Services (USCIS) as a civil surgeon to perform immigration medical examinations and to complete Form I-693 Report of Medical Examination and Vaccination Record. This examination is required of immigrant applicants within the U.S. seeking to change their status or obtain a benefit.
A complete medical evaluation and physical examination is performed as well as administration and review of required laboratory tests and vaccinations. After all requirements are met and applicant is medically cleared, Form I-693 is completed and provided in a sealed envelop to submit to USCIS.
Please bring with you a valid government issued picture I.D. (passport or driver’s license) and any vaccination records and medical reports in your possession. See “Preparing for your immigration physical” for more details. Only a medical doctor designated as civil surgeon can evaluate immigrants in the U.S. who are seeking to change their status. Furthermore, the examination needs to be carefully performed and Form I-693 properly completed as per the Technical Instructions For Civil Surgeons provided and updated by the Centers for Disease Control and Prevention (CDC).
