Services
Fitness for duty physical examination to certify that the Merchant Mariner applicant is of sound health and has no physical or mental limitations that would prevent him or her from performing the duties required to safely operate or work on a vessel. We will perform a complete medical evaluation and physical examination to evaluate if you are physically fit to perform the tasks necessary to function on a ship during ordinary and emergency conditions.
Merchant Mariner Medical Evaluation Form will be completed as well as all required testing.
Pre-employment DOT urine drug screen is performed (the drug testing fee is additional to the physical examination fee and is our standard fee charged for any DOT urine drug test).

The preferred method to submit your application and supporting documentation is via e-mail to MEDAIP@uscg.mil. Please include your full name in the subject line and ensure attachments are in PDF format.
